Please read the instructions for making an appointment as this will explain where to fax these forms as well as copies of driver's license, front and back of insurance card, and name, address and telephone number of your local physician so we can send reports to him/her.

Please arrive at least 15 minutes early for your appointment in order to make your visit with Orthopaedic Specialists of Texas as enjoyable as possible.
